Where do put screw bolts for the flooring

It comes with the screws and has holes in the plastic where they go. You will have to line them up with the floor joists.

Yes, each one is held down with 5 screws which are provided.

Has anyone used pins too install instead of screws? Easy removal for access

No. I had to remove and adjust. It was easy enough to use an electric drill to remove screws and reinstall flooring.

Somehow I misplaced the screws which came with these. What size and length are they?

They are just common drywall screws. I think 2 inches. Measure the thickness of the plastic and get something that goes down another 3/4 to 1 inch below that.
