Can I use this with Black & Decker BDCDMT120 20-volt Matrix Drill ?

Yes this attachment is made specifically for the Matrix Drill, I also believe the attachments themselves are interchangeable with the craftsman bolt-on drill as well

Can a reciprocating blade be used with this jigsaw attachment or is the reciprocating attachment required

no it only excepts jig saw blades

DOes it come with a blade or more than one?

It only comes with one blade. Its good quality so it should last.
