A: I did not purchase this. I purchased a flat piece for knee surgery. The ice did not stay cold very long. The ice cubes are surrounded by fabric, so if the hat is too large you could probably pin it together to make it smaller. Hope this helps.

A: Probably by hand in the sink then hang to dry or put under a ceiling fan?

I really, really like this elbow ice pack. I am using it on my left elbow. The only 2 things I'll mention: if you have very large arms, this may not fit you. Fits great and stays pretty cold. I will say I have never gotten 2 hours of use out of it. I have carpal tunnel and use it over my hands and then move it up my arm. By the forth move at 20 min each is just cool, not cold.
